If you would like to set up an appointment with Layne Physical Therapy please remember these important steps:

Get a Physical Therapy prescription if you will be using insurance from your physician. |

If your insurance requires a referral from your primary care physician (PCP), have your PCP’s referral coordinator (nurse) fax the Physical Therapy prescription if you will be using insurance/referral to Layne Physical Therapy to do your physical therapy. |

If you do not have insurance, a Physical Therapy prescription if you will be using insurance from your doctor is helpful but not required. If you are out of network, please consider private pay. |

After the Physical Therapy prescription if you will be using insurance is obtained, call Layne Physical Therapy 303-238-4277.

Please arrive 10 to 15 minutes early for your first visit to fill out the required registration paperwork (unless you have filled out the forms ahead of time by downloading our online forms). If you have a printer and would like to fill out the registration paperwork before your appointment, you may download the form and print it by visiting our forms page on this website. |

Please bring your insurance card and Physical Therapy prescription if you will be using insurance. You will be presented with a written verification of your insurance limitations, exclusion, and required co-pays and deductibles as well as your insurance percentage of responsibility. |
